This role has two primary focuses: providing NP-level hospice care to enrolled pediatric patients and supporting a specialist pediatric palliative care consultation service. The vast majority of visits will take place in the patient's home , with occasional visits to hospitalized pediatric patients as needed. Some visits may also be conducted via telehealth based on clinical appropriateness and patient needs. This role is ideal for a clinician with strong skills in advanced symptom management, family-centered communication, goals-of-care discussions, hospice eligibility review, and interdisciplinary teamwork.
