Location Address: 6100 Pan American Freeway NE, Albuquerque, NM 87109 Type of Opportunity: Full time FTE: 1.00 Overview Provider cares for patients, providing outpatient, primary care services focused on patient‑centric measures such as access to care, value‑aligned prescribing, diabetes care, and cardiovascular care. The provider will lead and work collaboratively with a team—including nurses, medical assistants, case/care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff—to deliver care. Panel size will remain small to ensure good access, reduced costs, and decreased reliance on the Emergency Department. Candidates must be versed in value‑aligned care. Practice Highlights Provide comprehensive medical care via in‑person, telehealth, and asynchronous encounters. Collaborate with social workers, community health, PHP benefits, and pharmacy. Orchestrate treatments, medications, and non‑pharmacologic therapy plans in partnership with patients and caregivers. Promote population health management programs and services to patients. See 14‑16 patients per day. Responsibilities Provide comprehensive medical care to patients with acute and chronic illnesses, focusing on access, cost management, and reduced ED and hospital utilization. Make professional medical judgments in the care of patients seen. Order, conduct, and interpret appropriate diagnostic x‑ray and lab tests. Prescribe treatments, medications, and non‑pharmacologic therapies with a strong alignment toward value. Document all patient encounters and communications appropriately. Manage the provider/patient relationship. Receive and resolve patient complaints. Involved in education and counseling of individuals, families, and groups. Serve as a healthcare resources, interdisciplinary consultant, and patient advocate. Work autonomously and in collaboration with other healthcare providers and staff as needed. Participate in recruitment and replacement of other providers. Participate in system‑wide health management initiatives. Provide input into the practice site’s financial, clinical, and service quality performance; budget, capital plan, and personnel issues. Provide clinical input into the development and review of appropriate clinical protocols and processes. Enhance professional growth and development through educational programs, literature review, in‑services, and workshops. Perform other functions as required. Attend and participate in all scheduled meetings including PCC, provider, and site meetings. Modify assessment techniques according to patient condition, culture, age, and stage of development. Maintain coding and encounter data standards for completeness, accuracy, timeliness, and reimbursement level, including legible documentation. Qualifications MD, Board Eligible or Certified in Internal Medicine or Family Medicine. State of New Mexico Medical License, DEA & NM Controlled Substance Registration.
