Unit Description: The Skilled Nursing Facility (SNF) at Champlain Valley Physicians Hospital is a 34-bed unit located on the 5th floor, serving a diverse population from middle-aged to geriatric patients. The unit maintains a balance of approximately 14 short-stay and 20 long-term residents. Short-stay patients typically remain 1 to 6 weeks and receive specialized care for conditions such as sepsis, post-operative orthopedic recovery, and wound care. Services include advanced wound management (including wound vacs), IV antibiotic therapy, and physical, occupational, and speech therapy. Long-term residents have a wide range of needs, from minimal assistance to full support with daily care. This unit offers the opportunity to build meaningful patient relationships while strengthening medical-surgical skills and participating in discharge planning to support successful transitions home.
