Our NP or PA will perform comprehensive health assessment which include but not limited to the following: Obtain history and perform physical examination Complete review of medicines and related instructions Review home safety and psychosocial needs Fulfill documentation needs for STAR and HCC measures Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies Maintain patient safety and confidentiality Universal precautions utilized in all patient care Prescribe/order procedures, treatments and medications within scope of practice Develop high level of trust and intimacy with patients and families The Skills, Experience & Education You Bring CA Licensed Nurse Practitioner or Physician Assistant 1 Experience with Primary Care You Share Our Mission & Values: Compassion We act with empathy and a deep respect for the challenges faced by physicians and their patients.
