This position will work three 12.5 hour rotating shifts a week, with day/night/swing shift rotation. We are currently seeking Advanced Clinical Providers (ACPs) to join our Hospital at Home (HAH) team. In this vital role within our Care Team Model, ACPs deliver high-quality, patient-centered care to patients receiving acute-level services in the home setting. Key Responsibilities: Provide clinical care to HAH patients under the direction of the Hospital at Home Medical Directors, Supervising Physician, and Associate Chief Advanced Clinical Provider Deliver high-quality clinical care to HAH patients across emergency departments, inpatient units, and the Virtual Unit. Assists the physician as directed in the training of health personnel in diagnostic, therapeutic and clinical techniques. Participate in continuing education and professional development activities to maintain clinical excellence. Shift: This is 24/7 service; three 12.5-hour shifts; MAINLY DAYS WITH MINIMAL NIGHTS (1-2 shifts a month) including weekends, and rotating holidays.
