Overview: Serving as a Home Health Aide CNA for hospice patients requires a uniquely compassionate person-someone who can manage daily care needs while providing a sense of comfort, stability, and normalcy within the patient's home. Practically, this role means working as part of the Hospice Interdisciplinary Group under the supervision of a registered nurse or licensed practical nurse. Demonstrates awareness and reinforces priorities in the plan of care. Encourages self-care and independence as appropriate. Identifies normal and abnormal mental function and demonstrates skills for addressing the needs and behaviors of the cognitively impaired. Cares for and maintains client's environment by avoiding environmental hazards, using restraints properly, and consistently using infection control procedures and barriers. Recognizes abnormal signs and symptoms of common disease conditions and communicates accurately and with timeliness. Provides and maintains basic nutrition and oral fluid intake. Maintain driver log (Manual or Peoplenet) according to DOT regulations, documenting delivery receipt, product temperatures and exceptions. Unload trailer, delivering product into customer premises.
