What You’ll Do Provide comprehensive inpatient care in a skilled nursing and rehabilitation setting Manage a panel of approximately 6–8 patients per day Assess and manage acute changes, chronic conditions, and post-hospital transitions Lead daily rounding, including review and triage of overnight events with nursing staff Support safe and efficient discharges back to primary care providers Collaborate closely with physicians, nurses, rehab therapists, and care managers Schedule 0.6 FTE (30 hours/week) Three 10-hour shifts per week (typically 7:00am – 6:00pm) Rotating weekend phone call (7:00 AM – 7:00 PM, from home); paid call coverage What You Can Expect Here A collaborative, interdisciplinary care model across a 150 bed facility Care for both long-term residents (90–100) and short-term rehab patients (50) A highly team-oriented environment with strong nursing and leadership support Meaningful continuity of care Qualifications NP or PA with (or eligible for) Vermont licensure BLS certification required Preferred: 2 years of experience in primary care, hospital medicine, urgent care, or post-acute care Comfort managing medically complex adult or geriatric populations Ability to work independently while collaborating closely with a multidisciplinary team Given the complexity of the patient population, candidates with prior APP clinical experience are strongly preferred Education Nurse Practitioner (NP) Graduate of a Nurse Practitioner program (Master’s or Doctoral degree) approved by the Vermont Board of Nursing or national accrediting agency Maintains active licensure as a registered nurse (RN) and active, unrestricted licensure to practice as an Advanced Practice RN (APRN) by the Vermont Board of Nursing RNFA certification strongly recommended DEA registration (or eligibility) Physician Assistant (PA) Successful completion of a Physician Assistant program at an institution accredited by ARC?PA (Accreditation and Review Commission on Education for the PA) Bachelor’s degree required; Master’s degree preferred Current certification by the National Commission on Certification of Physician Assistants (NCCPA) Active, unrestricted licensure to practice as a PA by the Vermont Board of Medical Practice DEA registration (or eligibility) Benefits Relocation assistance Medical, dental, and malpractice coverage 403(b) retirement plan Paid time off CME support If you’re looking to practice at the intersection of acute and long-term care where teamwork, continuity, and clinical variety come together—we encourage you to apply and join the Woodridge team at CVMC.
