Senior Registered Nurse - Oncology Services Clinic

Location: Moody Outpatient Center 3rd Floor Shift- 8 hours (8-5 PM) Employment type: Part Time with benefits *must be able to work a minimum of 24 hours per week* Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. Minimum Specifications Education Must be a graduate from an accredited school of nursing. Experience Must have two years of nursing experience. May participate in community outreach activities (such as health fairs, community collaborations or outreach clinics) as assigned. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern nursing practice, and ensures the hospital and Parkland are in compliance. This includes EMTALA, Order of Productive Custody, Memorandum of Transfer and APOWW. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge of sanitation, infection control, safety, supplies, equipment, appropriate utilization of supplies and materials. Performs appropriate patient assessments meeting admission and discharge criteria, provide safe medication administration using the patients six (6) rights, adequately perform POC monitoring and demonstrate safe infection control practices. Performs patient assessments related to cancer care and other medical diagnosis to include allergies and hypersensitivity reactions, physical, signs and symptoms, changes in activities of daily living, psychosocial, support network and financial dimensions across the continuum of care (diagnosis, treatment, surveillance, and end of life). Completes appropriate documentation by maintaining accurate and timely entries and notations in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of Oncology Services and Parkland. Educates patients, families and significant others on cancer care, therapies, treatment side effects, interventions and prevention. Performs patient teaching with documented care plans that are disease site oriented as related to phase of care across the continuum and for discharge planning. Collaborates with wound care stoma and ENT nurses for performing wound, colostomy and tracheotomy care as directed. Manages triage for patients/families/other internal and external health care professionals to optimally navigate the health care system. Seeks and utilizes direction from more experienced staff and team members as needed. May work with other staff in providing direction to non-nursing staff. Location: Parkland: 15th FLR BLK 400 Advanced Practice Provider II- ACLS Primary Purpose Responsible for providing health care for patients in designated primary or specialty areas, including patient diagnoses, treatment, education, and referrals. The Chief Psychiatrist provides clinical direction, general supervision, and training to the site staff, and has other managerial responsibilities. They approve, sign, and monitor the implementation and efficiency of all treatment plans. The Chief Psychiatrist creates discharge plans to insure appropriate follow-up and support after discharge from the facility. PSYCHIATRIST ESSENTIAL FUNCTIONS Provide onsite psychiatric assessment, diagnosis, and treatment of those inmates referred by the medical and clinical services staff.Provide written summaries of history, diagnosis and treatment course.Where applicable, provide psychiatric services for inpatients, assessment, diagnosis, staffing, treatment planning, and regular review of all inmates assigned.Where applicable, provide psychiatric services for all inmates referred to the Healthcare Unit for those inmates in the general population who need regular psychiatric/behavioral health follow up.Where applicable, provide psychiatric services for those inmates who need to be transferred to Behavioral Health facilities. Services to include work up and any court hearing testimony needed.Where applicable, act as regular Psychiatrist and participate in the treatment programming and planning for theBehavioral Health Unit (if applicable) duties include regular staffing and contact with Behavioral Health Unit staff and supervisor, discussion of problem cases, follow up and monitoring of medication, and general psychiatric input.Provide medication management to all inmates admitted.Provide individual and/or group therapy as appropriate.Approve, sign, and monitor the implementation and efficiency of treatment plans.Approve all admissions and discharges into the Behavioral Health Unit, if applicable.Provide in-service education to the site staff as needed.Attend training and meetings as required. Qualifications Licenses and Certifications: License and current registration to practice as a Registered Professional Nurse in New York State required Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required Valid driver's license, as determined by operational/regional needs may be required Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans required Maintains NPI, Medicaid and Medicare provider numbers preferred Maintains a collaborative practice agreement with a physician in compliance with New York State regulations preferred Must be certified by ANCC or another accrediting Nurse Practitioner body – in order to bill Medicare and meet credentialing requirements required For Psychiatric Nurse Practitioners only: Current PMHNP-BC certification required Education: Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) required Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required For Psychiatric Nurse Practitioners only: Master's Degree in psychiatric-mental health Nurse Practitioner required PhD in psychiatric-mental health Nurse Practitioner preferred Work Experience: Minimum of two years of experience as a nurse practitioner utilizing full scope of practice preferred Clinical home care experience or two years managerial experience preferred Demonstrated knowledge of Hedis and QARR quality measures, ICD-10 and CPT coding for reimbursement of services required Bilingual skills, as determined by operational needs required Pay Range USD $109,900.00 - USD $146,500.00 /Yr.
